REPORTING OF ACCIDENTS AND DANGEROUS OCCURENCES

  • All incidents, accidents or dangerous occurences MUST BE REPORTED by the QUICKEST MEANS POSSIBLE to each person’s respective supervisor. The supervisors must report all incidents, accidents and dangerous occurences to the mine’s safety department.
  • The scene of an incident, accident or dangerous occurrence must not be disturbed without permission, meaning that no item or equipment may be moved on the scene of an incident, accident or dangerous occurrence without the prior permission from the mine’s full time health and safety representative or the chief safety officer, provided that substantial evidence has been collected and an in-loco investigation completed.
  • Any person who fails to comply with the incident, accident and dangerous occurrence reporting procedure commits an offense and will be liable to disciplinary actions.
