The following entries shall be made:
- Name of person making the entry.
- For a group or team, the name of the team leader and a number of his/her team members.
- Reason(s) for entering the workplace.
- Time and date
Entry will be valid only if signed in.
The miner in charge shall check workplace register to ensure that persons who have made entries have signed out of the workplace before blasting time.
SAMPLERS
- The sampler team leader shall make an entry in Work Place register.
- On entry into a workplace, the miner will see the sampling crew and give safety instructions where necessary.
- On completion, the miner in charge of the workplace must check the Workplace Register to ensure the samplers has signed out.
- Where possible prior arrangements should be made with the responsible Miner or Shift Supervisor for Survey or Sampling employees visiting the section.
ENGINEERING WORKERS (Call outs or break downs)
- Engineering workers will be required to make an entry in the workplace register that will be kept at the miner’s box whenever they enter the underground workplaces for whatever reason.
VISITORS
- The HOD organizing the visit will ensure that the visitor(s) will undergo a medical examination and the attendance of induction course prior to the underground visit. Once the visitor(s) has been cleared by the OHC, the training center will upon completion of the training ensure that the visitors sign the indemnity form and provide a copy thereof to the visitor to be handed in at the specific mine.
- It is the responsibility of the mine official accompanying the visitors to make an entry into the register at the lamp room.
CONTRACTORS
- It is the responsibility of the Head of Department who has hired a contractor to ensure that the contractor complies with all the applicable procedures as stipulated in SOP 38: Outside Contractors.
- For permanent contractors e.g. Cementation Mining etc the same procedure as for engineering workers will apply.
OTHER PERSONS
- All persons going into a workplace for any reason shall be required to make an entry in the workplace register.
