Personal protective equipment (PPE) must be worn at all times – as indicated by symbolic signs or as per special instruction by the mine manager or competent person.
Understanding
- I understand that I fail to make use and correctly wear the personal protective equipment provided, witch may result in me sustaining an injury on duty, the Compensation Commissioner shall be informed of such failure
- In addition, I am fully aware that failure on my part to use the PPE is an offence. The use thereof is a legal requirement and it will be regarded as a contravention of the MHSA regulations
- I understand that I will be refused access to work if I arrive without the necessary PPE, which was issued to me
- I understand that should any PPE become lost or damaged due to my own negligence, I will be held responsible for the replacement thereof.
